Before submitting this form, please determine if you are eligible to enroll after open enollment begins (check the Academic Calendar for the first day newly admitted or readmitted students can enroll.) If you are returning from an official leave of absence with the university or are able to enroll in courses after open enrollment begins, you don't need to submit this form.
Students who are not continuously enrolled or on active leave of absence status (except for the summer semester) must be reactivated and pay the $50 reactivation fee. If a student has not enrolled in over a year, they must re-apply for admission.
